1.  Bachelor's degree in nursing;

2. Atleast 1 year of experience as a professional nurse in a hospital/community or public/private health agency.

3. Duly accomplished Application Form (available at the Graduate School Office or filled-up in the website) together with the following documents:

• Original copy of the official Transcript of Records/Transfer credentials;
• Two (2) recommendations from employer/s or school (forms included in the application packet);
• Certified true copy of college diploma;
• 4 passport-size photos (in white background);
• Filled-up form describing your motivation for pursuing graduate study and your view of self-directed learning as a method if instruction, and a description of your research interest;
• Copy of current nursing license (PRC);
• Copy of PSA birth certificate; and
• Copy of the marriage contract (if married);

• For Foreign Students
